Major Medical & ACA

What does major medical health insurance cover?

ACA-compliant major medical plans generally include essential health benefits such as preventive services, hospital care, emergency services, prescription drugs and mental health services. Your costs depend on the plan’s deductible, copays, coinsurance and network.

Can I get coverage if I have a preexisting condition?

ACA-compliant individual and small-group health plans cannot deny coverage or charge more because of preexisting health conditions. Other types of coverage may have exclusions or medical underwriting.

When can I enroll in an ACA Marketplace plan?

Most people enroll during the annual Open Enrollment Period. Certain life events, such as losing qualifying coverage, marriage or having a baby, may qualify you for a Special Enrollment Period.

Do I qualify for help paying my monthly premium?

Premium tax credit eligibility depends on factors including household income, household size, location and access to other qualifying coverage. We can review your estimated eligibility and available plans.

What is the difference between an HMO and a PPO?

HMOs generally use a more restricted provider network and may require referrals. PPOs typically offer greater flexibility for out-of-network care, often at a higher cost. Exact rules vary by plan.

Short-Term Medical

Is short-term health insurance the same as major medical?

No. Short-term coverage is temporary, is generally not ACA-compliant and may exclude preexisting conditions, preventive care, prescriptions or other services. Review the policy limitations carefully.

Who might consider short-term coverage?

People between jobs or waiting for other coverage sometimes consider short-term plans. They are not a substitute for comprehensive major medical coverage, particularly when ongoing care is needed.

Can a short-term policy be renewed?

Renewal and duration rules depend on federal and state requirements and the specific policy. Do not assume that a policy will renew or cover a condition that develops during a previous term.

Medicare

What is the difference between Medicare Advantage and Medicare Supplement?

Medicare Advantage (Part C) is a private-plan way to receive Medicare Part A and Part B benefits. Medicare Supplement (Medigap) works with Original Medicare to help cover certain out-of-pocket costs. You generally cannot use Medigap to pay Medicare Advantage costs.

When should I begin reviewing Medicare options?

It is wise to review options several months before turning 65 or retiring. Your enrollment deadlines may depend on whether you already have qualifying employer coverage.

Does Medicare cover dental, vision and hearing?

Original Medicare generally does not cover routine dental care, routine eye exams for glasses or hearing aids. Some Medicare Advantage plans include additional benefits, subject to plan rules and limitations.

Can I keep my doctors with Medicare Advantage?

It depends on the plan network, provider participation and any referral or prior-authorization requirements. We recommend checking your specific doctors, hospitals and prescriptions before enrolling.

When can I change my Medicare plan?

Medicare has several enrollment periods, including the Annual Enrollment Period from October 15 through December 7. Other opportunities may apply based on your current coverage and circumstances.

Do I need a separate prescription drug plan?

With Original Medicare, you may need a standalone Part D plan for prescription coverage. Many Medicare Advantage plans include drug coverage, but not all do. Coverage depends on the specific plan.

Group Health & Business Benefits

Can a small business offer group health insurance?

Many small businesses can offer group coverage, subject to carrier participation rules, employee eligibility and other requirements. We can help review traditional group and alternative arrangements.

What is an ICHRA?

An Individual Coverage Health Reimbursement Arrangement allows an eligible employer to reimburse employees for qualifying individual health insurance and, when permitted, certain medical expenses under a compliant arrangement.

Can employers contribute different amounts for employees?

ICHRA contributions can vary across certain permitted employee classes and by age or family size within applicable rules. Employers should use a qualified administrator or advisor to structure the arrangement correctly.

Can I offer dental and vision with group health benefits?

Yes. Depending on the carrier and employer eligibility, dental and vision benefits may be offered alongside medical coverage or separately.

Supplemental Insurance

What is supplemental health insurance?

Supplemental policies may pay cash benefits or reimburse certain costs for covered events such as accidents, hospitalization or specified critical illnesses. They are intended to complement, not replace, comprehensive health insurance.

How does a hospital indemnity plan work?

A hospital indemnity policy generally pays a defined benefit when covered hospitalization conditions are met. Benefits, waiting periods, exclusions and payment amounts vary by contract.

Does critical illness insurance cover every serious illness?

No. These policies cover only the conditions and events defined in the contract, subject to eligibility, exclusions and benefit limits.

Can I have supplemental insurance with Medicare or employer coverage?

In many cases, yes, although eligibility, coordination and suitability depend on the product and existing coverage. We can review how the policy would fit your situation.

Dental & Vision

Can I purchase dental and vision coverage separately?

Often, yes. Some carriers offer standalone dental or vision policies, while others bundle benefits. Availability depends on your state and the carrier.

Are major dental services covered immediately?

Not always. Some plans impose waiting periods for major services such as crowns, bridges or dentures. Check annual maximums, deductibles, networks and waiting periods.

What should I look for in a vision plan?

Compare exam copays, allowances for frames or contact lenses, lens benefits, provider networks and how often benefits renew.

Working With Our Agency

Does it cost more to use an independent insurance broker?

In many cases, using a broker does not increase the premium for the same insurance policy. Compensation arrangements vary by carrier and product, and we can explain how we are compensated.

Do you represent more than one insurance company?

Yes. As an independent agency, we can help compare products from multiple insurers where we are licensed and appointed. Not every carrier or plan is available in every location.

What information do I need for a quote?

Usually your ZIP code, ages or dates of birth, desired coverage type and relevant household or employer details. Some products require additional health or eligibility information.

Can you help me review my existing coverage?

Yes. We can review your current benefits, premium, provider network and potential gaps, then discuss available alternatives if appropriate.
